If you FORGET YOUR PASSWORD for access to A&E's PRISM Web, please fill out this request. You will need to know your USER NAME and the email address you used when your account was created. This is your "Registered" email address.
To prevent unauthorized access to the PRISM Web system new passwords can only be sent to your Registered email address.
